• $100 In-Office Visit-includes testing for strep, flu, COVID, and UTIs when appropriate

• $50 Indiana Telehealth Visit (no testing included)

  • You will be texted and emailed appointment link at time of your visit​

• Cash/Card Pay- We do accept HSA/FSA/Credit/Debit. However, we do not accept insurance​
