Terms and Conditions

By opting in to receive SMS messages from Huffman Health & Wellness dba/Freedom Medical Center (FMC) (“we,” “us,” “our”), you agree to these Terms and Conditions (Terms).

SMS Messaging Service:
By providing your phone number, you consent to receive SMS text messages from FMC, which may include appointment reminders, marketing messages, and general two-way communication
about Health and Wellness Services. Message and Data Rates may apply.

Message Frequency:
You may receive more than one message per month unless you opt out. While messaging frequency varies, you will typically receive up to two messages per month. FMC reserves the
right to modify the frequency of messages at any time. FMC and carriers are not liable for delays or undelivered messages.

Message and Data Rates:
Standard message and data rates may apply according to your mobile carrier’s terms.

Health Information & HIPAA Notice:
By opting in, you acknowledge that SMS messages may not be secure or encrypted and that there are risks in transmitting personal health information via text. FMC will limit health-related details in messages to protect your privacy, but you understand and accept the potential risks. Do not share sensitive health information via text. For questions about privacy, refer to our HIPAA Notice of Privacy Practices.

Two-Way Texting Rules:
● Text messages should not be used for urgent medical matters.
● Responses may not be immediate; for critical concerns, please call our office directly.
● Avoid sending sensitive personal health details via SMS.

Medical Emergencies:
Do not use SMS messaging for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

Opt-Out & Support:
You may opt out at any time by replying STOP to any message. For support, reply HELP or contact our office directly.
By continuing to receive SMS messages from FMC, you agree to these Terms.

Opt-In Policy for SMS Messaging:
Providing a proper opt-in process for customers is critical. Opt-in refers to how we collect your phone number and obtain your explicit approval to send SMS messages. The Campaign
Registry (TCR) requires businesses to obtain clear consent before sending text messages.

Key Opt-In Rules
● Rule #1 – We must obtain your explicit approval before sending SMS messages.
● Rule #2 – We must provide an option for you to opt out at any time.

Website Opt-In
If you provide your phone number through our website, we ensure transparency by clearly stating the following:
✔ You are opting in to receive SMS messages from us.
✔ You can opt out at any time by replying STOP.
✔ You can request assistance by replying HELP.
✔ Message and data rates may apply based on your carrier’s terms.
✔ Consent is not required as a condition to purchase goods or services.
✔ Our Privacy Policy and Terms of Service outline our texting practices.

How to Opt Out:
To stop receiving SMS messages, reply STOP to any message. You will receive a final confirmation message and will no longer receive further texts.
